The student health portal is where all students can go to fill out health forms, immunization forms, and upload required documentation. Students will be able to use the portal to communicate with the Clarke Health Center through the secure messaging feature.
Follow the link to the portal. Sign in with your Elmira College username and password. If your EC email is mtwain28@elmira.edu, your username is mtwain28. Students will select the "Forms" tab and will be able to see all forms that need to be completed and submitted. As the forms are completed, they will drop off the list. There is an embedded link for the physical exam form that students will need to print and then upload to the portal when completed.
